Usefulness of Serum Pepsinogen Test for Detection of Atrophic Gastritis in Health Check-Up Population

Abstract
Background/Aim : This study was performed to determine whether serum pepsinogen (PG) can be used to detect atrophic gastritis (AG).

Methods: AG was evaluated by endoscopy in 2,400 subjects. Serum PG I, II, and Helicobacter pylori IgG were measured. The risk of AG was evaluated in the relation to these markers.

Results: The best cut off value of the PG I/II for the AG was 4.0 with sensitivity of 50.3% and specificity of 72.0%. In univariate analysis, male, old age (£¾50), positive for H. pylori, and low PG were associated with AG. In multivariate analysis, both positivity for H. pylori (OR 2.59, 95% CI 2.17¡­3.10) and low PG (OR 2.81, 95% CI 2.22¡­3.56) were associated with AG. After subjects were classified into four groups based on H. pylori antibody and the level of PG, the risk of AG was highest in the H. pylori-positive, low PG group with an OR of 4.87 (95% CI: 3.74¡­6.33).

Conclusions: PG I/II ratio (¡Â4) was found to be reliable marker for the detection of AG. Patients with positive PG test who were positive for H. pylori IgG constitute a high-risk group for AG and a good candidate for endoscopic screening for gastric cancer.
